How The Online Form Works at Friern Barnet Medical Centre
The Process

Choose the right option - Visit our online form and select the button that matches your needs:
- "I have a health problem": Use this for any medical query, whether it is routine or urgent.
- Available: Monday to Friday, 8:00am – 6:30pm.
- "I have an admin query": Use this for fit notes (sick notes), test results, or letters (subject to Non-NHS Fees).
- Available: 24 hours a day, 7 days a week.
- "I have a health problem": Use this for any medical query, whether it is routine or urgent.

GP-led review - Your request is reviewed by a Doctor. The GP will carefully assess your needs and decide the best place for your care (such as an appointment, a prescription, or directing to an appropriate service).
- We contact you - We will contact you back with the outcome within 24 hours of you submitting the form.
- Please note: Any requests sent after 5pm will only be looked at the next working day so do not send anything urgent after 5pm. Contact us by phone or 111 instead
Support for patients with additional needs
We understand that not everyone can use online forms. If you have additional needs, please call us or attend in person. Our Reception team can:
- Teach you how to use the form yourself for next time.
- Fill out the online form on your behalf.
